Lion Air Flight 610 crashed after MCAS uncommanded inputs killed 189

On 29 October 2018, Lion Air Flight 610, a Boeing 737 MAX 8, crashed into the Java Sea shortly after takeoff from Jakarta, killing all 189 people on board. The NTSB investigation identified that the Maneuvering Characteristics Augmentation System (MCAS) could generate uncommanded flight control inputs, and the design assumed immediate pilot corrective action. The NTSB issued safety recommendations to the FAA regarding design enhancements and pilot training. A similar crash of Ethiopian Airlines Flight 302 occurred on 10 March 2019.
