The NHTSA has launched a preliminary investigation into the aftermarket driver-assistance device openpilot after it was involved in five collisions that resulted in three deaths and up to 11 injuries. The system, which costs about $999 and can be self-installed, failed to detect or respond to slow or stationary vehicles in the same lane. One fatal crash in Louisiana involved a Toyota RAV4 running a third-party software branch that struck a stopped police vehicle, killing two rear-seat passengers.
