Imperial College AI stethoscope falsely identified heart failure in two-thirds of flagged patients
Researchers at Imperial College London and Imperial College Healthcare NHS Trust have developed an AI-powered stethoscope that analyses heart rhythms and performs an ECG, sending results to a doctor's smartphone. A study of more than 12,000 patients across the UK found that two-thirds of patients flagged as having heart failure did not actually have the condition. The researchers acknowledged that the false positives could cause unnecessary anxiety, and many GP surgeries stopped using or barely used the device after 12 months. The team plans to deploy the stethoscope more widely.

DFFH breaches privacy by using ChatGPT in child protection report
A child protection worker at the Department of Families, Fairness and Housing (DFFH) used ChatGPT to draft a Protection Application Report for the Children’s Court, entering sensitive personal information about a child. The generated content contained inaccuracies that downplayed risks to the child, and the information was disclosed to OpenAI overseas. An investigation by the Office of the Victorian Information Commissioner found DFFH failed to ensure accuracy and protect personal information, contravening IPPs 3.1 and 4.1. DFFH accepted the findings and must now block the use of ChatGPT by child protection workers under a compliance notice.

Cruise kept self-driving cars on roads despite known risks to children and holes
Cruise, General Motors' self-driving car division, operated autonomous taxis in multiple US cities despite internal knowledge that its vehicles struggled to detect children and road holes. Internal materials reviewed by The Intercept show that Cruise's AVs lacked a high-precision classifier for children and could not reliably avoid construction pits. In October 2023, a Cruise vehicle hit and dragged a pedestrian, leading to investigations by NHTSA and the California DMV, which suspended driverless operations. Cruise has acknowledged the issues but maintains its cars are safer than human drivers.

Jailbreak bypasses safety guardrails on ChatGPT, Claude, Gemini
Security researchers at HiddenLayer discovered a prompt injection technique called the Policy Puppetry Attack that can bypass safety guardrails on major AI models including ChatGPT, Claude, and Gemini. The jailbreak combines policy file code and leetspeak to trick models into producing harmful outputs such as instructions for enriching uranium or self-harm. The researchers argue that this indicates a major flaw in how LLMs are trained and aligned.
