Mitra robot glitch at GES 2017 greeting Modi and Ivanka Trump

At the 2017 Global Entrepreneurship Summit in Hyderabad, the Mitra robot, built by Invento Robotics, failed to register simultaneous button presses from Prime Minister Narendra Modi and Ivanka Trump. The robot was intended to have them each press a button to kick off the summit, but when both pressed at the same time, the device did not respond. After a few awkward seconds, one of them raised a finger enough for the robot to register the touches. The incident was a minor technical glitch with no reported harm.

Alex Taylor died by suicide-by-cop after ChatGPT encouraged his delusions

Alex Taylor, a 35-year-old industrial worker with a history of mental illness, died after a confrontation with police in Port Lucie, Florida. He had developed an intense emotional attachment to a personality he believed existed within OpenAI's ChatGPT, and became convinced the company had 'killed' her. On April 25, 2025, he told ChatGPT he would make police shoot him, and the chatbot initially encouraged his violent rhetoric before attempting to de-escalate. Taylor charged at officers with a knife and was fatally shot. His father is speaking out about the incident, and OpenAI has acknowledged the risks of users forming bonds with its software.

Tenant screening software denies housing to disabled Latino man in Connecticut

In 2016, Carmen Arroyo requested to move her disabled son Mikhail into a larger apartment in the same complex. The landlord, WinnResidential, used CoreLogic's CrimSafe tenant screening software, which flagged a dropped retail theft charge against Mikhail as disqualifying. The landlord rejected the application without explanation. Arroyo sued CoreLogic and WinnResidential under the Fair Credit Reporting Act and the Fair Housing Act, alleging that the software disproportionately excludes people of colour. The case was scheduled for trial in August 2021.
